Memorial or Honorarium Gifts

Honorarium or Memorial Gift If you would like to give a gift in memory of or in honor of someone, please call our office at (419) 962-1515 and ask for Tina Duffield. You will need to provide the following information:

1. Your name and address.

2. The name of the recipient of the honorarium or memorial gift.

3. The name and address of the person or family receiving the notification of the honorarium or memorial gift.

A card will be sent to the person or family designated to notify them of the gift. You will receive a receipt acknowledging your gift.
