Barbara's Questionnaire

#1 – Give a one or two paragraph description of your childhood, good or bad.

#2 – Do you remember the first time you thought about leaving the Amish culture/faith?

#3 – What led up to that moment? Was it multiple incidents or one major one?

#4 – Give the details about how you made the decision to leave.

#5 – In reference to #4, tell me about your plans to leave, your feelings about leaving, and whether you told your parents/family/friends?

#6 – Where did you go when you left and did you know anyone who was able to help you make the transition.

#7 – Describe if you can in a paragraph or two the first six months after leaving…..how easy was the transition for you?

#8 – Describe visiting your parents and family/friends for the first time after leaving if you have done so and what your relationship is with them currently.

#9 – Share briefly your lowest point since leaving and then also your highest point.

#10 – Give me 3 successes or achievements you have accomplished since leaving the Amish.

#11 – Share with me your dream…………..what do you want to do or see happen in your life?

#12 – If you have put your faith and trust in Jesus Christ would you please share your testimony of how that came to pass?

#13 – Please feel free to add any additional information you want to share or share this questionnaire with anyone else you think may be interested in sharing their story.
